      Incontinence as the last taboo

While many things cannot be changed in the health life of a frail senior, urinary incontinence is no longer one of them. A great many women are surprised to learn that urinary incontinence is a common and treatable condition. And, contrary to yet another myth, it is not a natural part of aging: it can happen at any age and can be caused by many physical conditions.

For many frail seniors urinary incontinence, or the inability to control the elimination of urine, has caused frustration and a loss of personal dignity.

What is not widely understood is that Incontinence is not a disease but rather a symptom of other conditions. And once the cause or causes of these conditions is determined, urinary incontinence can be treated.

Our treatment for frail seniors begins with a thorough assessment of their current situation. Following a history and physical examination, urinalysis with possible urine culture, and catheterization may follow. Female clients will have a pelvis exam to determine the condition of the tissue and to check if the internal organs (uterus, bladder or rectum) have dropped. Assessment for incontinence determines the presence or absence of acute causes such as infection and urethral obstruction, and includes an evaluation of functional ability and cognitive status, both of which are important in determining successful interventions.

Because there are many forms of chronic incontinence, we ask our clients to keep a bladder diary which helps us get a clear picture of the individual's symptoms. Regardless of the cause, there are successful medicine-based and surgical treatments available. These may not be suitable for the frail senior, for whom surgery is too risky and some medications may cause undesirable side effects. For our frail senior clients behavioral interventions are highly successful in managing incontinence. The reported success rate for behavioral treatment is 85%.

This history of success should encourage those people suffering from incontinence
to realize that not only can we help them, but that with a success rate that high they are likely to have a positive outcome. Together we will work toward a return to a normal lifestyle that will be no longer hampered by incontinence.
